Medicine information: Thiamine Hydrochloride
Indications
Vitamin B1 for:
- Treatment and prevention of thiamine deficiency (beriberi)
- Wernicke's encephalopathy and its prevention in alcohol dependence, malnutrition, prolonged vomiting (hyperemesis), after bariatric surgery
- Peripheral neuritis due to deficiency
- Nutritional supplementation
Dosage & administration
Mild deficiency / prevention (oral): 10–25 mg daily; in alcohol dependence 100 mg two to three times daily (oral absorption is limited).
Severe deficiency: 200–300 mg daily in divided doses.
Wernicke's encephalopathy (hospital): high-dose IV thiamine (e.g. 200–500 mg three times daily for several days), given before glucose, then oral maintenance.
Children (deficiency): 10–50 mg daily in divided doses.
- Injections only by health professionals.
Contraindications
- Allergy to thiamine (rare; mainly with injections)
Side effects
Oral: very well tolerated; rarely stomach upset.
Injection: pain at the injection site; rarely severe allergic reactions/anaphylaxis (especially with fast IV injection), so it should be given slowly with resuscitation facilities.
Precautions & warnings
- In malnourished or alcohol-dependent patients, give thiamine before glucose-containing fluids to avoid precipitating Wernicke's encephalopathy.
- Oral thiamine is not a substitute for IV treatment in suspected Wernicke's encephalopathy.
Pregnancy & breastfeeding
Safe in pregnancy and breastfeeding at recommended doses; used in hyperemesis gravidarum.
Interactions
- Alcohol reduces absorption
- Loop diuretics (furosemide) may increase thiamine loss with long-term use
- No significant harmful interactions
Overdose
Excess is excreted in urine; oral overdose is unlikely to be harmful. Large IV doses may cause allergic reactions.
Storage
Store below 30°C, protected from light.
