Cacetor Capsule

Combination of a calcium-channel blocker and an ACE inhibitor for high blood pressure not controlled by either alone.

Adults: one capsule once daily — amlodipine 2.5/5/10 mg + benazepril 10/20/40 mg; usually start 5/10 or 2.5/10 mg (elderly, liver impairment); maximum 10/40 mg daily.

• Same time each day, with or without food.

• History of angioedema (with ACE inhibitors or hereditary)
• Pregnancy
• Use with sacubitril/valsartan (within 36 hours), or with aliskiren in diabetes/kidney impairment
• Severe aortic stenosis, shock
• Allergy to dihydropyridines or ACE inhibitors

Common: dry cough, ankle swelling, dizziness, headache, flushing, tiredness.

Less common: low blood pressure, palpitations, raised potassium, raised creatinine, nausea.

Rare but serious: angioedema (face/tongue swelling — emergency), kidney failure, liver problems, low white cells.

• Check kidney function and potassium periodically.
• Avoid dehydration; caution in renal artery stenosis and heart failure.

Contraindicated in pregnancy. Not recommended while breastfeeding.

• Potassium supplements, spironolactone, trimethoprim (high potassium)
• NSAIDs (reduced effect, kidney injury)
• Lithium (toxicity)
• ARBs, aliskiren, sacubitril (avoid)
• Simvastatin (limit 20 mg); CYP3A4 inhibitors raise amlodipine
• Insulin/diabetes medicines (hypoglycaemia)

Severe low blood pressure, fast heartbeat. Emergency care.

Store below 30°C, protected from moisture.
