Indications

First-generation oral cephalosporin for:

Streptococcal sore throat and tonsillitis
Skin and soft-tissue infections (impetigo, cellulitis, boils, wound infections)
Urinary tract infections
Bone and joint infections (follow-on treatment)

Dosage & administration

Adults: 500 mg–1 g twice daily (or 1 g once daily for skin infections and strep throat); maximum 4 g/day.

UTI: 1 g twice daily (or 1–2 g once daily for uncomplicated cystitis).

Children: 30 mg/kg/day in 2 divided doses (or once daily for strep throat and impetigo); maximum adult dose.

Guide (suspension 125 or 250 mg/5 ml): under 1 year 25–50 mg/kg/day; 1–6 years 250 mg twice daily; over 6 years 500 mg twice daily.
Strep throat: 10 days.
Kidney impairment (CrCl < 50): longer dosing interval.
With or without food.

Contraindications

Allergy to cephalosporins
Severe immediate allergy to penicillins

Side effects

Common: nausea, diarrhoea, indigestion, rash, itching.

Less common: vomiting, vaginal thrush, headache.

Rare but serious: anaphylaxis, severe skin reactions, C. difficile colitis, liver inflammation, blood disorders, kidney inflammation.

Precautions & warnings

Ask about penicillin/cephalosporin allergy.
Kidney impairment: adjust dose.

Pregnancy & breastfeeding

Considered safe in pregnancy and breastfeeding.

Interactions

Probenecid (raises levels)
Warfarin (monitor INR)
Aminoglycosides, loop diuretics (kidney effects at high doses)

Overdose

Nausea, vomiting, diarrhoea. Supportive care; haemodialysis removes it.

Storage

Store below 30°C, dry. Reconstituted suspension: keep in a fridge and use within 14 days.
