Indications

Serotonin and histamine antagonist for:

Prevention of migraine and cluster headache (not for treating an acute attack)
Sometimes used as an appetite stimulant (off-label)

Dosage & administration

Adults: start 0.5 mg at night, increase gradually to 1.5 mg daily (single bedtime dose or three divided doses); maximum 4.5 mg daily (single dose not more than 3 mg).

Children 5 years and over (migraine prevention, specialist): up to 1.5 mg daily, usually 0.5–1 mg at bedtime.

Benefit may take several weeks; review after 3–6 months.

Contraindications

Allergy to pizotifen
Use with MAO inhibitors (caution)

Side effects

Very common: increased appetite and weight gain, drowsiness.

Common: dizziness, dry mouth, nausea, tiredness, muscle pain.

Less common: depression, anxiety, constipation, difficulty passing urine.

Rare: seizures, liver problems, aggression in children.

Precautions & warnings

Drowsiness — do not drive if affected; avoid alcohol.
Narrow-angle glaucoma, urinary retention, epilepsy, kidney impairment: caution.
Monitor weight.

Pregnancy & breastfeeding

Avoid in pregnancy unless clearly needed. Avoid while breastfeeding.

Interactions

Alcohol, sedatives, antihistamines (more drowsiness)
Adrenergic neurone blockers (guanethidine) — reduced effect
Anticholinergic drugs (more side effects)

Overdose

Drowsiness, dizziness, nausea, low blood pressure, seizures, coma. Seek medical care.
