Indications

Sulphonylurea medicine for type 2 diabetes:

  • Lowers blood sugar when diet, exercise and weight loss are not enough
  • Used alone or with metformin, other tablets or insulin
Dosage & administration

Immediate-release (80 mg): start 40–80 mg daily with breakfast; increase gradually; doses above 160 mg are split into two; maximum 320 mg/day.

Modified-release (30/60 mg MR): start 30 mg once daily with breakfast; increase every 4 weeks if needed; maximum 120 mg once daily.

  • Take with breakfast; swallow MR tablets whole (the 60 mg MR can be halved if scored).
  • Do not skip meals after taking it.
  • Elderly and kidney impairment: start low.
Contraindications
  • Type 1 diabetes, diabetic ketoacidosis, diabetic coma
  • Severe kidney or liver impairment
  • Allergy to gliclazide or other sulphonylureas/sulphonamides
  • Use with miconazole (oral)
  • Pregnancy and breastfeeding (insulin is used instead)
Side effects

Most important: low blood sugar (hypoglycaemia) — sweating, shaking, hunger, fast heartbeat, confusion; more likely if meals are missed, with alcohol, exercise, kidney/liver disease or other diabetes drugs.

Common: stomach pain, nausea, indigestion, diarrhoea or constipation.

Less common: weight gain, rash, itching.

Rare: liver problems, jaundice, blood disorders, severe skin reactions, haemolytic anaemia (G6PD deficiency).

Precautions & warnings
  • Learn the signs of low blood sugar; carry sugar or glucose.
  • Regular meals; extra care with alcohol, fasting (e.g. Ramadan — dose adjustment needed), heavy exercise.
  • G6PD deficiency: risk of haemolysis.
  • Monitor blood sugar and HbA1c regularly.
Pregnancy & breastfeeding

Not recommended in pregnancy or breastfeeding; insulin is preferred.

Interactions
  • Miconazole, fluconazole (severe hypoglycaemia)
  • Beta-blockers (mask hypoglycaemia), ACE inhibitors, clarithromycin, NSAIDs, fluoroquinolones, alcohol (increase hypoglycaemia)
  • Steroids, thiazides, chlorpromazine, danazol (raise blood sugar)
  • Warfarin (effect may increase)
Overdose

Severe, prolonged low blood sugar (may last days) — give sugar if conscious and seek emergency care; hospital observation needed.
