Indications
SGLT2 inhibitor for:
- Type 2 diabetes in adults (with diet and exercise, alone or with other medicines)
- Reducing the risk of cardiovascular death in type 2 diabetes with heart disease
- Heart failure (reduced or preserved ejection fraction), with or without diabetes
- Chronic kidney disease (slowing progression), with or without diabetes
Dosage & administration
Adults: 10 mg once daily in the morning, with or without food.
- Type 2 diabetes: may be increased to 25 mg once daily if more sugar control is needed (and eGFR is 60 or above).
- Heart failure and chronic kidney disease: 10 mg once daily.
Kidney function: for blood-sugar lowering, start only if eGFR ≥ 20–30 (sugar effect is weaker at low eGFR); for heart and kidney protection 10 mg may be used down to eGFR 20.
- Not for children under 10 years; type 1 diabetes not approved.
Contraindications
- Allergy to empagliflozin
- Type 1 diabetes or diabetic ketoacidosis
- Dialysis
Side effects
Common: genital fungal infections (itching, discharge — especially women and uncircumcised men), urinary tract infections, passing more urine, thirst, raised cholesterol.
Less common: dehydration, dizziness, low blood pressure (elderly, on diuretics), low blood sugar with insulin or sulphonylureas, itching.
Rare but serious: ketoacidosis even with near-normal blood sugar (nausea, vomiting, stomach pain, fast breathing — emergency), Fournier's gangrene (painful swelling around the genitals with fever), severe kidney infection.
Precautions & warnings
- Stop 3 days before planned surgery, and during acute illness, fasting, vomiting or diarrhoea (sick-day rules).
- Drink enough fluids; elderly and people on diuretics may become dehydrated.
- Keep the genital area clean and dry.
- Check ketones if unwell, even if sugar is normal.
Pregnancy & breastfeeding
Not recommended in pregnancy (especially 2nd and 3rd trimesters) or while breastfeeding.
Interactions
- Loop and thiazide diuretics (dehydration, low blood pressure)
- Insulin, sulphonylureas (low blood sugar — their dose may need lowering)
- Lithium (levels may fall — monitor)
- Urine glucose tests are positive while taking it (expected)
Overdose
Increased urine output and dehydration; low sugar if combined with other diabetes drugs. Supportive care.
