Dosage & administration
Adults (oral): 250–500 mg every 6 hours, or 500 mg–1 g every 12 hours; up to 4 g daily in severe infections.
Adults (injection, IM/IV): 2–4 g daily in 4 divided doses; up to 8 g daily in severe infections.
Children (oral): 25–50 mg/kg/day in 2–4 divided doses (up to 100 mg/kg/day for ear infections), as prescribed.
- Can be taken with or without food.
- Reduce the dose in kidney disease.
Contraindications
- Allergy to cephradine or other cephalosporins
- History of severe (anaphylactic) reaction to penicillins
Side effects
Common: nausea, diarrhoea, abdominal discomfort.
Less common: rash, itching, thrush (oral or vaginal), headache, dizziness.
Rare: severe allergic reaction, C. difficile colitis (severe diarrhoea), blood disorders, raised liver enzymes. Pain at the injection site with IM use.
Precautions & warnings
- About 10% of people allergic to penicillin may also react to cephalosporins.
- Use a lower dose in kidney impairment.
- Can give a false-positive urine glucose test (copper-reduction method).
Pregnancy & breastfeeding
Generally considered safe in pregnancy when needed. Passes into breast milk in small amounts — usually acceptable; watch the baby for diarrhoea or thrush.
Interactions
- Probenecid (increases cephradine levels)
- Loop diuretics (e.g. furosemide) and aminoglycosides — possible increased kidney toxicity
- Warfarin (may increase bleeding risk)
Overdose
Nausea, vomiting, stomach pain and diarrhoea; seizures are possible at very high doses, especially in kidney failure.
Storage
Store below 30°C, dry and away from light. Reconstituted suspension: refrigerate and use within 7 days. Use reconstituted injection promptly.
