Indications

Anti-anginal metabolic agent used as:

Add-on treatment of stable angina in adults not adequately controlled by or intolerant of first-line anti-anginal drugs

Dosage & administration

Adults: modified-release 35 mg twice daily with breakfast and dinner (or immediate-release 20 mg three times daily).

Moderate kidney impairment (CrCl 30–60) and elderly over 75: 35 mg once daily (in the morning).
Review after 3 months; stop if no benefit.

Contraindications

Parkinson's disease, parkinsonian symptoms, tremor, restless legs syndrome and related movement disorders
Severe kidney impairment (CrCl < 30)
Allergy to trimetazidine

Side effects

Common: dizziness, headache, abdominal pain, diarrhoea, indigestion, nausea, rash, weakness.

Serious: parkinsonism (tremor, stiffness, slowness), gait instability, restless legs — usually reversible after stopping; falls in elderly; rarely low platelets, liver problems.

Precautions & warnings

Stop if movement disorders appear; review by a neurologist if they persist after 4 months off the drug.
Not for treating an acute angina attack or as initial treatment of unstable angina/heart attack.
May cause dizziness — caution with driving.

Pregnancy & breastfeeding

Avoid in pregnancy and breastfeeding.

Interactions

No major interactions known. Use cautiously with drugs that cause dizziness or falls.
