Indications
Short-acting bronchodilator (reliever) for:
- Relief of wheezing and breathlessness in asthma and COPD
- Prevention of exercise- or allergen-induced bronchospasm
- Nebuliser: severe acute asthma attacks
- Tablets/syrup: when inhalers cannot be used (less preferred)
Dosage & administration
Inhaler (100 mcg/puff): adults and children — 1–2 puffs as needed for symptoms, up to 4 times daily; before exercise 2 puffs. In an attack, more puffs may be needed on medical advice.
Nebuliser: adults 2.5–5 mg up to 4 times daily (more often in severe attacks under supervision); children 2.5 mg.
Tablets/syrup: adults 2–4 mg three or four times daily; children 2–6 years 1–2 mg, 6–12 years 2 mg three or four times daily.
- Use a spacer with the inhaler for better delivery, especially in children.
- If you need your reliever more than 2–3 times a week, see your doctor — asthma control may need a preventer.
Contraindications
- Allergy to salbutamol
- Oral forms: threatened miscarriage or to delay premature labour (not recommended)
Side effects
Common: shaky hands (tremor), headache, fast heartbeat, palpitations, nervousness.
Less common: muscle cramps, mouth/throat irritation.
Rare: low potassium (high doses), irregular heartbeat, paradoxical bronchospasm (wheeze worsens right after use — stop and use another reliever, seek help), allergic reactions.
Precautions & warnings
- Use with caution in heart disease, high blood pressure, overactive thyroid, diabetes (blood sugar may rise).
- High doses with steroids, diuretics or theophylline can lower potassium.
- Seek urgent help if the reliever is not working as well as usual.
Pregnancy & breastfeeding
Inhaled salbutamol is safe to use in pregnancy and breastfeeding to control asthma.
Interactions
- Non-selective beta-blockers such as propranolol (block its effect, may trigger asthma)
- Diuretics, steroids, theophylline (low potassium)
- MAO inhibitors, tricyclic antidepressants (heart effects)
- Digoxin (low potassium raises toxicity)
Overdose
Fast heartbeat, tremor, agitation, low potassium, high blood sugar, chest pain. Seek medical care.
Storage
Store below 30°C. Do not puncture or burn the inhaler canister, even when empty. Protect nebuliser solution from light.
