Adults and children over 12 years: 500 mg–1 g every 4–6 hours as needed. Maximum 4 g (8 × 500 mg) in 24 hours.
Children: 10–15 mg/kg every 4–6 hours, maximum 4 doses (60 mg/kg) in 24 hours. Use the dosing chart on the pack for syrup/drops; measure with the provided spoon or dropper.
Suppository: dose by age/weight as above.
- Leave at least 4 hours between doses.
- Do not take with other paracetamol-containing products.
- Lower maximum (e.g. 2–3 g/day) in liver disease, heavy alcohol use, low body weight or poor nutrition.
Contraindications
- Allergy to paracetamol
- Severe active liver disease
Side effects
Side effects are rare at normal doses.
Rare: skin rash, allergic reactions, blood disorders (low platelets or white cells).
Very rare but serious: severe skin reactions (SJS/TEN) — stop and seek help if a blistering rash appears.
Liver damage can occur with overdose or regular use above the maximum dose.
Precautions & warnings
- Do not exceed the maximum daily dose — many cold/flu medicines also contain paracetamol.
- Use with caution in liver or kidney disease, alcohol dependence, malnutrition and dehydration.
- See a doctor if fever lasts more than 3 days or pain more than 5 days (children: more than 3 days).
Pregnancy & breastfeeding
The preferred painkiller in pregnancy and while breastfeeding when used at the lowest effective dose for the shortest time.
Interactions
- Warfarin (regular high-dose paracetamol may raise INR)
- Enzyme-inducing drugs (carbamazepine, phenytoin, rifampicin) and alcohol — higher liver risk
- Metoclopramide/domperidone (faster absorption); cholestyramine (reduced absorption)
Overdose
Medical emergency even if the person feels well. Early symptoms (nausea, vomiting, pale skin) may be mild; liver damage appears after 1–3 days. Go to hospital immediately — the antidote (acetylcysteine) works best within 8 hours.
Storage
Store below 30°C in a dry place, out of reach of children.
