Medicine information: Ferric Carboxymaltose
Indications
Intravenous iron for:
- Iron deficiency anaemia when oral iron is ineffective, not tolerated, or rapid replacement is needed (e.g. inflammatory bowel disease, chronic kidney disease, heart failure with iron deficiency, late pregnancy, before surgery, heavy bleeding)
Dosage & administration
Given in hospital/clinic by IV infusion or slow injection, with monitoring for 30 minutes afterwards.
- Total iron need is calculated from body weight and haemoglobin; usually 500–1,000 mg per dose (maximum 20 mg/kg per dose, 1,000 mg per week).
- Typical adult: 1,000 mg in one infusion (≥ 15 minutes), a second 500–1,000 mg dose after 1 week if needed.
- Children 1 year and over: 15 mg/kg per dose (max 750 mg).
- Recheck haemoglobin and ferritin 4 weeks after the last dose.
Contraindications
- Anaemia not caused by iron deficiency
- Iron overload
- Known serious allergy to IV iron products
- First trimester of pregnancy
Side effects
Common: headache, dizziness, nausea, flushing, raised blood pressure, injection-site reactions (including long-lasting brown skin staining if the drug leaks), low blood phosphate (can cause fatigue, bone pain, fractures with repeated doses).
Less common: rash, itching, fever, taste changes, muscle aches.
Rare but serious: severe allergic (anaphylactic) reactions — given only where resuscitation is available.
Precautions & warnings
- Observe for at least 30 minutes after each dose.
- Do not give with oral iron (no benefit).
- Check phosphate if repeated courses or symptoms of weakness/bone pain.
- Asthma, eczema, multiple allergies: higher risk of reactions.
- Active infection (bacteraemia): delay treatment.
Pregnancy & breastfeeding
Avoid in the first trimester; may be used in the 2nd and 3rd trimesters when oral iron is not suitable. Compatible with breastfeeding.
Interactions
- Oral iron — absorption reduced; start oral iron at least 5 days after the last injection if needed
- No other significant interactions
Overdose
Iron overload is possible with excessive doses — monitored by ferritin.
Storage
Store below 30°C; do not freeze. Use promptly after dilution in sodium chloride (only diluent allowed).
