Gabapentinoid (α2δ ligand) for neuropathic pain:

• Diabetic peripheral neuropathic pain
• Post-herpetic neuralgia (pain after shingles)
• Peripheral neuropathic pain from other causes (e.g. spinal stenosis, cervical/lumbar radiculopathy)

Adults: start 5 mg twice daily; increase by 5 mg per dose at intervals of at least 1 week to 15 mg twice daily (range 10–15 mg twice daily).

Kidney impairment:

• Moderate (CrCl 30–59): start 2.5 mg twice daily, target 5–7.5 mg twice daily.
• Severe (CrCl below 30): start 2.5 mg once daily, target 5–7.5 mg once daily.
• Take with or without food.
• Do not stop suddenly; reduce over at least 1 week.

• Allergy to mirogabalin

Common: drowsiness, dizziness, swelling (oedema) of legs, weight gain, unsteadiness.

Less common: headache, constipation, blurred vision, tiredness, dry mouth.

Rare but serious: loss of consciousness, falls, liver dysfunction, misuse/dependence, breathing depression with opioids.

• Do not drive or do hazardous work — dizziness and sleepiness are common.
• Elderly: higher risk of falls.
• History of drug misuse: monitor.
• Watch for mood changes.

Limited data; use in pregnancy only if benefit outweighs risk. Avoid breastfeeding during treatment.

• Alcohol, opioids, benzodiazepines, sedating antihistamines (more drowsiness, breathing depression)
• Probenecid, cimetidine (raise levels)
• Lorazepam (additive effects)

Drowsiness, dizziness, unsteadiness, loss of consciousness. Supportive care; haemodialysis may help.
