Medicine information: Misoprostol
Indications
Prostaglandin E1 analogue used for:
- Prevention and treatment of postpartum haemorrhage (when oxytocin is not available)
- Obstetric and gynaecological uses under medical supervision (as per national guidelines): induction of labour, management of miscarriage, cervical preparation before procedures
- Prevention and treatment of NSAID-induced stomach and duodenal ulcers
Dosage & administration
Obstetric uses must only be done under medical supervision according to national guidelines.
- Postpartum haemorrhage prevention: 600 micrograms by mouth immediately after the birth of the baby (when oxytocin is unavailable); treatment: 800 micrograms under the tongue.
- NSAID ulcer prevention: 200 micrograms two to four times daily with food (with the NSAID).
- NSAID ulcer treatment: 800 micrograms daily in 2–4 divided doses for 4–8 weeks.
Contraindications
- Pregnancy when continuation is wanted (for ulcer prevention/treatment) — can cause miscarriage, premature birth and birth defects
- Allergy to prostaglandins
- Previous caesarean section or uterine scar (for labour induction — higher risk of uterine rupture; specialist decision)
Side effects
Common: diarrhoea, abdominal pain, nausea, vomiting, wind, headache; shivering and fever (especially after obstetric doses).
Gynaecological: uterine cramps, vaginal bleeding.
Serious: heavy bleeding, uterine rupture (in labour, especially with a scar), severe dehydration from diarrhoea.
Precautions & warnings
- Women of child-bearing age using it for ulcers must use effective contraception and must not be pregnant.
- Heart or blood-vessel disease, inflammatory bowel disease, dehydration: caution.
- Keep out of reach of others; must not be used without medical supervision in pregnancy.
Pregnancy & breastfeeding
Contraindicated in pregnancy for non-obstetric uses. Obstetric use only under medical supervision. Small amounts pass into milk — may cause diarrhoea in the baby; usually compatible after postpartum doses.
Interactions
- Magnesium-containing antacids (worsen diarrhoea)
- Oxytocin (enhanced uterine effect — monitor)
- NSAIDs (used together intentionally for ulcer protection)
Overdose
Sedation, tremor, seizures, breathing difficulty, abdominal pain, diarrhoea, fever, palpitations, low blood pressure, slow heartbeat. Seek emergency care.
Storage
Store below 25–30°C in the original blister, protected from moisture (degrades in humidity).
