Indications
Neurotropic B vitamins (B1, B6, B12) for:
- Nerve pain and tingling/numbness (peripheral neuropathy), e.g. diabetic or alcohol-related, sciatica, neuralgia
- Deficiency of vitamins B1, B6 and B12
- Supportive treatment in back and neck pain with nerve involvement
- Injection: severe deficiency, as directed by a doctor
Dosage & administration
Adults: 1 tablet once to three times daily, or as directed (commonly 100 mg B1 + 200 mg B6 + 200 mcg B12 per tablet).
Injection: 1 ampoule IM daily to 2–3 times weekly, as prescribed.
- Take after meals.
- Long-term high-dose use (B6) should be supervised.
Contraindications
- Allergy to any of the vitamins (especially cobalt/cobalamin)
- Leber's disease (hereditary optic nerve atrophy) — cyanocobalamin
- Use of levodopa alone
Side effects
Usually well tolerated.
- Occasional nausea, stomach upset, diarrhoea.
- Red/pink or dark urine colour is possible.
- Rare: allergic reactions, acne-like rash, itching (more with injections).
- Prolonged high-dose B6 (over 100–200 mg/day for months) can cause nerve damage (numbness).
Precautions & warnings
- Do not exceed the prescribed dose for long periods.
- B12 can mask folate deficiency in megaloblastic anaemia — proper diagnosis needed.
- Injections: rare severe allergic reactions.
Pregnancy & breastfeeding
High-dose combinations are not recommended in pregnancy and breastfeeding unless prescribed; standard-dose B vitamins are safe.
Interactions
- Levodopa (pyridoxine reduces effect unless with carbidopa)
- Isoniazid, penicillamine, oral contraceptives (increase B6 needs)
- Metformin, PPIs, colchicine (reduce B12 absorption)
Overdose
Low toxicity; long-term B6 excess causes neuropathy.
Storage
Store below 25–30°C, protected from light and moisture.
