Telcare-40 Tablet

Angiotensin-II receptor blocker (ARB) for:

• High blood pressure
• Reducing cardiovascular risk (heart attack, stroke, cardiovascular death) in high-risk adults (established vascular disease or diabetes with organ damage)

Hypertension: 40 mg once daily (20 mg may be enough for some); may be increased to 80 mg once daily.

Cardiovascular prevention: 80 mg once daily.

• Take at the same time each day, with or without food.
• Full effect in 4–8 weeks.
• Mild–moderate liver impairment: maximum 40 mg daily.

• Pregnancy
• Biliary obstruction, severe liver impairment
• Use with aliskiren in diabetes or kidney impairment
• Allergy to telmisartan

Common/uncommon: dizziness, back pain, sinusitis, upper respiratory infection, diarrhoea, low blood pressure, raised potassium, raised creatinine, muscle cramps.

Rare but serious: angioedema, kidney failure, sepsis (reported), severe low blood pressure, liver problems.

• Check kidney function and potassium periodically.
• Avoid dehydration; caution with renal artery stenosis, heart failure, aortic stenosis.
• Diabetes on insulin: may lower sugar slightly.

Contraindicated in pregnancy — switch before planned pregnancy. Not recommended while breastfeeding.

• Potassium supplements, spironolactone, trimethoprim, salt substitutes (high potassium)
• NSAIDs (reduced effect, kidney injury)
• Lithium (toxicity)
• ACE inhibitors, aliskiren (avoid combining)
• Digoxin (levels may rise — monitor)

Low blood pressure, fast or slow heartbeat. Lie down with legs raised; seek medical care.

Store below 30°C, in the original blister (protect from moisture).
