Indications

Liver-directed thyroid hormone receptor-β agonist for:

Non-cirrhotic metabolic dysfunction-associated steatohepatitis (MASH/NASH) with moderate to advanced liver fibrosis (F2–F3), with diet and exercise (specialist use)

Dosage & administration

Adults: by body weight, once daily with or without food:

Under 100 kg: 80 mg once daily
100 kg and over: 100 mg once daily
With moderate CYP2C8 inhibitors (e.g. clopidogrel): under 100 kg 60 mg, 100 kg and over 80 mg.
Continue lifestyle changes (weight loss, exercise, no alcohol).

Contraindications

Decompensated cirrhosis (not studied/recommended)
Use with strong CYP2C8 inhibitors (gemfibrozil)
Allergy to resmetirom

Side effects

Common: diarrhoea, nausea (mostly early), itching, abdominal pain, vomiting, constipation, dizziness.

Less common: raised liver enzymes (hepatotoxicity — monitor), gallbladder-related events (gallstones, cholecystitis).

Precautions & warnings

Monitor liver tests; stop if signs of liver injury.
Report right-upper abdominal pain, fever, jaundice (gallbladder or liver problems).
Thyroid disease: may affect thyroid tests; specialist review.

Pregnancy & breastfeeding

Limited data; avoid in pregnancy unless clearly needed. Not recommended while breastfeeding.

Interactions

Gemfibrozil (contraindicated); clopidogrel (reduce dose)
Statins: limit doses (e.g. rosuvastatin/simvastatin max 20 mg, pravastatin/atorvastatin max 40 mg)
CYP2C8 substrates (e.g. pioglitazone, repaglinide) — monitor

Limited experience; supportive care.
