Indications

Combination of a calcium-channel blocker and an ARB for high blood pressure not controlled by either alone, or to replace separate tablets.

Dosage & administration

Adults: one tablet once daily — amlodipine 5 mg + valsartan 80 mg, 5/160, 10/160, 5/320 or 10/320 mg.

Increase after 1–2 weeks if needed; maximum 10/320 mg daily.
With or without food, same time daily.
Not for children under 18.

Contraindications

Pregnancy
Severe liver impairment, biliary cirrhosis or cholestasis
Shock, severe aortic stenosis, unstable heart failure after a heart attack
Use with aliskiren in diabetes or kidney impairment
Allergy to dihydropyridines or valsartan

Side effects

Common: ankle swelling, dizziness, headache, flushing, tiredness, nasopharyngitis.

Less common: low blood pressure, palpitations, raised potassium/creatinine, joint pain, cough.

Rare but serious: angioedema, kidney failure, severe low blood pressure, liver problems.

Precautions & warnings

Check potassium and kidney function periodically.
Avoid dehydration; caution in heart failure, renal artery stenosis.

Pregnancy & breastfeeding

Contraindicated in pregnancy. Not recommended while breastfeeding.

Interactions

Potassium supplements, spironolactone, salt substitutes (high potassium)
NSAIDs (reduced effect, kidney risk)
Lithium (toxicity)
ACE inhibitors, aliskiren (avoid combining)
Simvastatin (limit 20 mg); clarithromycin, itraconazole, grapefruit (raise amlodipine)

Overdose

Severe low blood pressure, fast heartbeat. Emergency care.
