Indications

Second-generation oral cephalosporin for:

Sore throat and tonsillitis (streptococcal)
Ear infections (otitis media), sinusitis
Bronchitis flare-ups
Uncomplicated skin and soft-tissue infections

Dosage & administration

Adults and children 13 years and over: 500 mg once daily or 250–500 mg twice daily for 10 days (skin: 250–500 mg twice daily or 500 mg once daily).

Children 6 months–12 years: otitis media 15 mg/kg twice daily; sinusitis 7.5–15 mg/kg twice daily; pharyngitis (2–12 years) 7.5 mg/kg twice daily — for 10 days (max adult dose).

Kidney impairment (CrCl < 30): half the dose.

With or without food.

Contraindications

Allergy to cephalosporins
Severe immediate allergy to penicillins

Side effects

Common: diarrhoea, nausea, abdominal pain, nappy rash/vaginal thrush, dizziness.

Less common: rash, itching, raised liver enzymes.

Rare but serious: anaphylaxis, serum-sickness-like reaction, severe skin reactions, C. difficile colitis.

Precautions & warnings

Ask about penicillin allergy.
Suspension contains sugar/phenylalanine in some products.
Complete the course.

Pregnancy & breastfeeding

Considered safe in pregnancy and breastfeeding when needed.

Interactions

Probenecid (raises levels)
Aminoglycosides (kidney effects)
Oral typhoid vaccine (reduced effect)

Overdose

Nausea, vomiting, diarrhoea. Supportive care; haemodialysis removes it.

Storage

Store below 30°C. Reconstituted suspension: refrigerate and use within 14 days.
